Student Athletes are asked to provide their personal insurance information on SWOL.  Additionally, a copy of the insurance card (front and back) is required. Student athletes are responsible for ensuring that this information is current.

The Chapman University Athletic Department provides secondary insurance coverage through Mutual of Omaha Specialty Risk for all injuries sustained while participating in intercollegiate athletics. 

To ensure coverage of this policy, certain steps must be followed:

  1. You must see providers that are in network (either for your insurance and/or for Mutual of Omaha).
  2. Chapman University athletic training (via SWOL) must have documentation, including a copy of the insurance card for your primary coverage.
  3. Your primary insurance must be billed first.  The primary carrier must provide an explanation of benefits (EOB) or a denial of coverage to the provider when bill(s) are received. 
  4. A copy of the EOB and all bills must be provided to Mutual of Omaha (as outlined in the attached document) or to the Director of Athletic Training at Chapman University.
  5. If you do not have insurance coverage, then the Mutual of Omaha policy becomes your primary for the care you receive for this injury.
  6. All information requested by the provider, the insurance company, and/or Chapman University must be provided upon request.
  7. All treatment for this injury must be completed no later than two years from the date of injury.

Failure to follow these steps could delay payment or alter your eligibility for benefits. As the patient, you and/or your parent, guardian, or spouse are ultimately responsible for the payment of all medical bills resulting from this injury. If you are not the policy holder for your primary insurance, you are encouraged to forward this information to the primary insurance policy holder.

      You must have a Southern California Kaiser Number

      If you have Kaiser from anywhere besides So Cal please request a So Cal number and add this information to the SWOL insurance tab

Medicaid, Tri-Care, HSA or CSP, Out of Area Plans, HMO Plans -

      These plans may prevent or slow our ability to secure appropriate care in the event of an athletic injury

      These plans may prevent or slow your ability to get appropriate care in the event of other medical issues

      Consider the CUSH health plan to ensure you have medical coverage while away from home
